Premature Ejaculation

Premature ejaculation (PE) is a common sexual dysfunction where a man ejaculates more quickly during sexual activity than he or his partner would like. It’s estimated that about 20-30% of men experience PE at some point in their lives, making it one of the most frequent male sexual issues. PE can lead to distress, embarrassment, and strain in relationships, affecting both the man’s and his partner’s sexual satisfaction.

Causes

The causes of PE can be complex and vary from person to person. They are generally categorised into psychological and biological factors:

  1. Psychological factors: These include performance anxiety, stress, depression, relationship issues, or a history of sexual trauma. Negative experiences and feelings about sex can also contribute to PE.
  2. Biological factors: Hormonal imbalances, abnormal levels of neurotransmitters (brain chemicals), inflammation or infection of the prostate or urethra, and genetic factors can play a role in PE. Additionally, thyroid problems or erectile dysfunction can be associated with the condition.
Treatment Options for Premature Ejaculation

PE is a treatable condition, and several approaches can help manage it effectively. Treatment options may include:

Behavioral techniques: Techniques such as the “stop-start” method or the “squeeze” technique can help men learn to delay ejaculation. These methods involve training the body to recognize and control the sensations leading to ejaculation.

Counseling and therapy: Psychological counseling, especially cognitive-behavioral therapy (CBT), can address underlying psychological causes like anxiety or relationship problems. Therapy may involve both the individual and their partner.

Types of Premature Ejaculation

PE is usually classified into two types:

  • Lifelong (primary) PE: This type has been present since a man’s first sexual experiences. It is often linked to genetic or neurobiological factors.
  • Acquired (secondary) PE: This develops later in life, often due to psychological stress, medical conditions, or changes in sexual function.

    Medications: Certain medications can help delay ejaculation. Selective serotonin reuptake inhibitors (SSRIs) such as sertraline, paroxetine, and fluoxetine are commonly used. These drugs increase the levels of serotonin, a neurotransmitter that plays a role in ejaculation control. Another option is topical anesthetics, such as lidocaine-prilocaine cream, which can be applied to the penis to reduce sensitivity.

    Pelvic floor exercises: Strengthening the pelvic floor muscles through specific exercises can improve control over ejaculation.

    Lifestyle changes: Reducing stress, improving communication with a partner, and addressing other health issues like erectile dysfunction or anxiety can also contribute to managing PE.
